A support referral begins a conversation about the assistance someone wants. This guide explains how to prepare an enquiry, consider permission and communication preferences, and use Holistic Health Support’s online referral form without assuming availability has already been confirmed.

A referral is a way to start a conversation about support. You can refer yourself, or make a referral for someone who has given you permission or for whom you have authority to act.

Prepare a support referral with permission

Before referring another person, make sure they understand what information you will share and why. Discuss who they would like us to contact about the enquiry.

Provide useful contact details

Follow the referral form’s instructions and provide the contact information requested. Check spelling and contact information so we can reach the right person.

Explain the support requested

Select the service or services of interest. You can choose personal care, daily living assistance, community and social support, in-home care or travel and transport assistance. If you are unsure, select that option and explain what you would like to explore.

Keep the first referral focused

A short explanation of support needs and preferences is enough to begin. Only share information you have permission or authority to provide. Contact our team if you have a question about the information requested. More detailed arrangements can be discussed if support is suitable.

What happens after submitting?

The referral is submitted through the embedded Jotform form for review. The nominated person can then be contacted to discuss needs, location and availability. Submitting a referral does not confirm that a service is available or that support has been agreed.

Ready to start?

Open the referral form to refer yourself or someone you support. If you prefer to talk first, call 0481 712 533.

Support referral: common questions

What is the purpose of an initial support referral?

An initial referral starts a conversation about the assistance requested, the person’s goals and practical availability. It gives the provider a starting point for an enquiry. It should not be treated as confirmation that a service is available or that support arrangements have already been agreed.

What if I am referring someone else?

Confirm that you have their permission or authority to share the information. Where possible, include their own words about what they want from support and who they would like contacted. Follow the form’s instructions and ask the team if you are unsure what information is needed.

Can I ask a question before filling in the form?

Yes. You can contact Holistic Health Support to discuss the services you want to explore and ask about the referral process. A brief description of the goal, suburb and assistance requested helps focus that conversation. Share information through the appropriate contact or form channel.

A referral that helps us understand the person

Holistic Health Support welcomes enquiries from participants, families, carers and professionals through our referral form. A useful referral explains the person’s goals as well as the assistance being requested. Include the nominated contact, location and preferred way to communicate so the next conversation can focus on what matters.

If you are referring someone else, confirm that you have their permission or authority to share their details. Where possible, include the person’s own words about what they want from support. Their choices, culture and everyday preferences provide useful context beyond the name of a service.

You can use our disability support services to identify the areas you want to discuss. For example, explain whether the enquiry relates to personal care, help at home, getting involved in the community or travel. If more than one service is relevant, describe the main priorities rather than assuming every type of support needs to begin together.

Holistic Health Support may be a good fit if you want a personal conversation before deciding on arrangements. A submitted referral starts that conversation and does not confirm service availability. If you prefer to begin with a question, talk with our team. Our guide to your first support conversation can help you prepare.

Bring these points to your conversation

  • Confirm consent or authority when referring someone else.
  • Describe the person’s goals and preferences.
  • Include their suburb and nominated contact.
  • Identify the services you want to discuss and any timing needs.
